September 05, 1869  Avondale  Plymouth 110 Fire -- A fire broke out and cut off the air supply to the miners, causing them to smother. The burning breaker at the top of the mine shaft made it impossible for anyone to escape. Because of this accident, new laws required that breakers could not be located over mine shafts. There were no laws preventing children as young as ten years old from working in the mines. That is why many of the victims of the Avondale disaster were children.

November 03, 1888  Kettle Creek  Cook's Run 17 Dynamite Explosion -- The drill post gave way and fell on a supply of dynamite and caps that had just been brought into the mine. The explosion was propagated by coal dust throughout most of the mine and up the airshaft. Five of the victims working in the vicinity of the dynamite explosion were killed by violence. Twelve others died from suffocation and "afterdamp".

October 11, 1905  Hazel Kirk No 2  Monongahela  Explosion -- About 2:30 p.m., Tuesday, an explosion occurred in the Hazelkirk mine No. 2, near this place and resulted fatally to two men who were in the mine at the time of the explosion. The mine was set on fire. 
October 13, 1905  Clyde  Fredericktown  Fire --  October 14, 1905 Fate of the Six Entombed Men Is Not Known boss bethought himself of six men who were at work to a room in the furthermost part of the mine, fully a half mile from the shaft. 
October 29, 1905  Hazel Kirk No 2  Monongahela  Explosion --  October 30, 1905 Because the output of coal was not large enough From Hazelkirk mine No. 2, near the village of Van Voohis, six miles from Monongahela, six experienced men went into the pit Saturday night to open up a bricked entry. An explosion of fire damp killed five of them instantly and injured one so severely that he may not recover. This is the second explosion within three weeks, the other on October 10, causing the death of two miners.
